breaks and lunches in an 8 hour work day

1. Is an employee that works a regular 8 hour shift,required by law to take a lunch break? 2. Can an employee take their 30 minute lunch breaks (or 15 minute) breaks at their own desks?

Can an employer have you punch out for your breaks. To monitor your lenght of time the emplyoee is taking?

It is my understanding that if you work for 8 hours; then you get 2 15 minute breaks... paid for. And one 30 minute lunch; not paid. Or you can get one hour, not paid.
