The website of the Yor History & Theory of Psychology graduate programme can be found at: http://www.psych.yorku.ca/grad/ht/welcome.htm. It contains the kind of information you want. The other major history of Psych program in North American is at the University of New Hampshire. There is some information about it at http://www.unh.edu/psychology/grad_pro.htm#history. I do not know that there are any other graduate programe specifically in the history of psychology, though there are some schools that will accept dissertations in the history of psychology if there are qualified professors at that school.

I found a valuable a valuable book written by Philip Cushman. The title is: "Constructing the self, constructing America, A cultural history of psychotherapy" This may add to your findings. Thanks
