Are there any boilerplate SLAs available for major ERPs for each of the modules they contain.../

Answers

You should not write SLAs for modules within applications or even for the applications themselves. You need to look at the service that is being delivered to the user. If you want to write specs for components in that service delivery system, then they belong in OLAs. Even then, the same principles for metrics should be applied.
