Get the 420EX, unless you need manual control, or a master for future slave flashes. The 420 is fully compatible with the Elan II. The 380EX was introduced with the Elan II, but it had a couple of glaring flaws. Manu people name the lack of a swivel head a the primary flaw, but I find the focus assist light to be worse. I only works with the center sensor, and only if it is manually selected, not when it is selected by eye control. I think the 420EX has the same AF assist light as the 550EX, and I know the 550EX works with all focus points on the Elan II because I use one on mine.

The 420EX works only a regular on camera flash, or a wireless slave. It cannot be used as a master for another 420EX being used as a slave. To use it as a slave, you need either the ST-E2, a 550EX, or an MR-14EX. The 550EX or course offers much more power, and full manual control as well.
