AU: Loose wire causes rail chaos

An estimated 150 passengers were kept on board
a stalled suburban train for an hour yesterday after
an overhead wire fell on its roof in the latest mishap
to befall the beleaguered CityRail system.

. . .

In the more serious incident, at Concord West, a
train travelling from Hornsby to Strathfield went
past a red signal, set up to protect a group of rail
maintenance workers, at an estimated 80km/h.

Official reports revealed that standard trackside
safety equipment failed to trip the train's brakes. It
took the train driver, with passengers aboard,
almost 300 metres to pull up.
