I believe the Nikon Coolpix 990 would be an excellent choice. Based on alot of research I've done recently on digital cameras, this camera is the one to get and it has the capability to close-focus at a distance of less than one inch from your subject.

The 990 is an excellent choice. For simplicity of storage though you may want to consider a camera like the Sony FD95. With it's floppy disk storage you can easily take several pictures and immediately label the disk for the particular patient and drop it in the file. The camera's viewfinder is through-the-lens which might be easier if you prefer the eye-piece approach to shooting. But the LCD on the rear of the 990 will work too (the eyepiece will present a paralax problem on the 990 in extreme macro work). If you haven't looked at the 95 you should at least take a quick looksy - otherwise the 990 is excellent. The 990 will also give you better resolution - but I'm not certain it's necessary for what you are shooting.
The cheap floppies as media that can be used for permanent storage in the patients files would seem attractive to consider.
