I got a camera for chrismas and I took some pictures with it and there is a chip that I put in to a disk to put them on my computer and it worked the first two times but now I can not get it to work. could you give me somethings that I can do to try and fix it or should I just send it back to the company? The model number of the camera is D-340R and the disk number is MAFP-2E

That's a "FlashPath" adapter, made by SmartDisk corp, distributed by various mfrs including Olympus. I'd look first for other software you've installed on the computer since first using the FlashPath. Maybe try re-installing the FlashPath's driver Next, make sure that the batteries inside the FlashPath are fresh. Third, try Olympus tech support, or see the SmartDisk website to see if they have any troubleshooting suggestions: http://www.smartdisk.com
