since factfinder and cl are so nonchalant regarding manual mode i thought i would try and find some examples of why the idea makes my hair stand on edge.

Purpose The U.S. Nuclear Regulatory Commission (NRC) is issuing this information notice to alert addressees to the potential for personnel errors during infrequently performed evolutions that result in, or contribute to events such as the inadvertent draining of water from the reactor vessel during shutdown operations.

then again under description of circumstances:

Operations staff practices including poor communications, poor activity briefings for high-risk activities, lack of effective pre-shift briefings, inadequate supervision of important control room activities, inadequate monitoring of control room panels, and slow event response may have contributed to the event.

so i have a suggestion. we might not have engineering degrees, but that does not invalidate ones ability to analyze data and come to logical conclusions.

let us all make a concerted effort to update this forum whenever we happen upon any information regarding operator errors...it should prove enlightening.

btw, does anyone have a url that 'breaks down', no pun intended, the series events that caused the meltdown at TMI?
