I am a student at Harvard Extension School, and a taking a class on teams with Dr. Harold Langlois. We are planning a study on a team system in the Health Care Profession, and are looking for any diagnostic questionnaires or advice regarding this topic, and how best to ask questions in order to elicit information. We are especially interested in the topic of transition between teams (ie. when a patient's treatment ends and the team is disbanded and focuses their attention elsewhere). Any assistance is greatly appreciated.
