I am trying to compose a model release form but I have no real clue on how to write one or what should be in it. So if anyone has any sort of adivice or would like to e-mail a copy of their own so I can have a good idea that would much appreciated.

Here is a very basic model release:

I hereby give the photographer the right to reproduce and publish my photograph. I am over eighteen (18) years of age.

Date:___________ Signature Name Address

As guardian of the above minor, I approve this release on his/her behalf.
