I've developed some components which may be of use in your application. The solution I have come up with is for imaging a 13x20mm area, specifically the iris of the eye. I have both film and video cameras with 4" working distance. I do not use a megapixel camera because for this type of macro bio-photography, focusing is easier using the live monitor rather than peering through a viewfinder. For best image quality I use RGB video and a Matrox framegrabber, and the 'viewfinder' is a custom program for viewing the image, making captures, labeling and storing to disk. For practical reasons, the image capture size is 640x480x24 RGB TIFs. I also use custom fiberoptic lighting. The system could easily be adapted to dental work. More info can be found at Miles Research.
