As an experienced recruiter who wants to "go it alone", I'm curious as to whether anyone has any particularly cunning/creative ideas in terms of service differentiation, candidate sourcing, recruitment advertising, referral schemes, fee structures etc.

Yes Murray I do have an idea, but being cunning isn't one of them. If I was to go out and create what you call a great sales recruitment company, I would instantly realize that I am not in the recruiting business but in the life making business. Then I will help people to obtain the jobs that would help them make their life and to enhance and add life to the living corporations they should be working for.
