Patch Adams Poem

hi, i'm looking for a poem that was in the movie Patch Adams. I'm not sure if was made up for the script or actually a "famous" poem I have never heard before.

The lines ( of what i can remember, and may not be exact) I have are:

I love you without knowing how, when, or from where. I love you straight forward, without complexities or... I love you so close that your hand on my chest is my hand...

That's all I have. I would appreciate any answers anyone can give me. If there is a full poem like this out there, I would really love to read the entire work and be familiar with the author! Thanks!
